Google has begun the rollout of its Health Guardian feature suite for the Pixel Watch 5, alongside the Pixel Watch 3 and 4, starting the week of September 24, 2026. These features provide long-term tracking for blood pressure trends and insulin resistance using passive sensors, removing the need for manual calibrations or traditional medical peripherals like blood pressure cuffs.
The first monthly health trend reports are scheduled to arrive in the Google Health app—the rebranded version of the Fitbit app—starting in early October 2026. While these features are free for Pixel Watch owners, users of the Fitbit Air will require a Google Health Premium subscription to access the trend data.
Passive Monitoring and AI Foundations
Google’s approach to blood pressure tracking differs from competitors like Samsung, which typically requires monthly calibration against a physical cuff. Instead, Google utilizes an AI foundation model known as WavesFM to analyze physiological signals for cuffless blood pressure monitoring.
For metabolic health, the suite includes Insulin Resistance Trends powered by the SensorFM model. This AI was trained on more than 1 trillion minutes of data from 5 million users to detect physiological signatures of metabolic stress. This allows the watch to track shifts in metabolic health over multiple weeks without requiring blood draws or specialized glucose monitors.
Data Requirements and Eligibility
To ensure accuracy, both features have specific data collection thresholds before they can generate a report:
- Blood Pressure Trends: Requires at least five consecutive days of data within a single week.
- Insulin Resistance Trends: Requires a minimum of seven days of data collected within a one-month period.
The update also includes Breathing Emergency Detection, which is designed to identify and respond to significant respiratory changes. However, this specific feature remains restricted to the Pixel Watch 4 and Pixel Watch 5 and is currently only available in select European markets. All other Health Guardian features are rolling out more broadly across the supported Pixel Watch and Fitbit hardware starting this week.
